The American marketplace usually offers us plenty of buying choices — sometimes it seems too many. A case in point: the toothpaste aisle at your local supermarket.
It can be a bit overwhelming with all the razzle-dazzle packaging and exciting claims of “Whiter Teeth!” or “Fresher Breath!” But toothpaste really isn't that complicated, if you keep in mind its primary goal: to help you with your toothbrush remove disease-causing plaque from teeth surfaces.
And the vast majority can, thanks to ingredients you'll find in just about every brand. All toothpastes, for example, contain some form of abrasive material that boosts the mechanical action of brushing to remove plaque. This isn't new: the ancient Egyptians used ox-hoof ashes, burnt eggshells and pumice as abrasives. Today you'll find hydrated silica (originating from sand), hydrated alumina or calcium carbonate as abrasives on the ingredient list.
You also need some form of detergent to help loosen and break down substances that won't dissolve in water. Toothpaste detergent is much milder than that which you use on your dishes. The most common is sodium lauryl sulfate, a foaming agent found in shampoo and other beauty products. It's been used safely for half a century in toothpaste, although it can irritate the inner linings of some people's mouths. If this is a problem for you, you should look for toothpaste with a different detergent.
There is also a myriad of other ingredients, including binders, humectants (which help the toothpaste retain moisture) and flavorings. You may also find bleaching agents that help brighten your teeth, although they may not be strong enough to remove deep staining, something we would need to help you with.
And let's not forget one other frequent ingredient: fluoride. This natural chemical strengthens enamel and helps fight tooth decay as part of a disease prevention strategy. It's perhaps the most valuable ingredient you'll find in toothpaste, so make sure it's in your chosen brand.
If you want to simplify your decision, choose toothpaste with the seal of acceptance from the American Dental Association. The seal indicates the claims of the toothpaste manufacturer have been independently verified. You can trust those brands to help keep your teeth clean and free from disease. In the end, that's really what you want from your toothpaste.

Although we’ve made great strides over the last century making dental visits more pleasant and comfortable, many people still feel a little apprehension about them at one time or another. For a few, though, this apprehension escalates into high anxiety — so high they may even avoid important dental treatment altogether.
If you have a significant phobia regarding dental visits and treatment, here are some things you can do to reduce your anxiety and feel more comfortable when you undergo treatment.
Let us know about your feelings of anxiety. We’re conditioned by society to regard such fears as irrational or “silly,” and so we tend to hide our negative emotions. Dentists, however, have been trained to work with fearful patients to reduce their anxiety levels. Being honest with us about your fears and nervousness is the first step to developing an anxiety-reducing strategy that will make your visits more pleasant.
Counteract bad experiences with good. For most people the fear they have during dental visits stems from earlier unpleasant experiences at the dentist. The fear can be so ingrained that simply trying to convince yourself or to be told “there’s nothing to be afraid of” will have little to no effect. Instead, build a memory collection of positive and pleasant dental visit experiences that serve to counteract the unpleasant. To do this we might first get you acclimated to routine visits and then gradually transition to more invasive procedures. This may increase the normal time for dental treatment, but the reduction in anxiety is worth the extra time.
Consider sedation therapy. In addition to modifying your experiences, you may also benefit from sedation medications that reduce anxiety, especially in the early stages of treatment. Depending on your medical history and current status, we can prescribe a sedative for you to take an hour or so before your appointment to help you relax. We can also increase the level of anesthesia (from local to intravenous or gas anesthesia, for example) if your anxiety is especially acute.
Taking proactive steps to minimize dental visit anxiety will increase the probability that you’ll obtain needed dental care. Your teeth and gums will be healthier for it.

It's difficult to measure how x-ray imaging has transformed dentistry since its use became prominent a half century ago. As equipment and methods standardized, the technology revolutionized the way we diagnose tooth decay and other mouth-related issues.
One of the more useful of these methods is called the bitewing x-ray. The term comes from the shape of the device a patient holds between their teeth with the film attached on the side toward their tongue. We direct the x-ray beam to the outside of the patient's cheek, where it passes through the teeth to expose on the film. Its particular design provides clearer images since the patient's bite helps keep the film still and distortion-free, making it easier to view signs of early tooth decay.
Bitewing x-rays usually consist of four films, two on either side of the mouth, necessary to capture all of the teeth (children with smaller jaws, however, often only require one film per side). How frequently they're conducted depends on a number of factors, including the patient's age: children or young adolescents are usually filmed more frequently than adults, usually every six to twelve months. Frequency also depends on a patient's particular decay risk — the higher the risk the more frequent the x-ray.
Regardless of how often they're performed, a similar application principle applies with bitewing x-rays as with any other radiological method: As Low As Reasonably Achievable (ALARA). With the ALARA principle in other words, we're looking for that sweet spot where we're able to detect the earliest stages of dental disease with the least amount of radiation exposure.
Bitewings fit this principle well: a patient receives only a fraction of the radiation exposure from a four-film bitewing as they do from a daily dose of environmental radiation. Factor in new digital technology that reduces exposure rates and bitewings pose virtually no health risk to patients, especially if conducted in a prudent manner.
The benefits are well worth it. Thanks to bitewing x-rays we may be able to diagnose decay early and stop it before it causes you or your family member extensive tooth damage.
